Nasal decontamination by iodine reduces severity of corona infection: Study
SARS-CoV-2 initially infects ciliary cells of the nasal mucosa, which becomes the ideal base of infection. A nasal decontamination by iodine reduces the risk to the patient by decreasing the viral load going to the lungs by deactivating them in the nasal mucosa, the source. Since PVP-I rapidly inactivates SARS-CoV-2, it can also play an adjunctive role in mitigating viral transmission to other people. Droplets and aerosols will be less infective.

NEW DELHI, Sept 20: Transnasal viral inactivation may not only prevent person-to-person spread of SARS-CoV-2, but may also diminish the severity of disease in patients by decreasing viral load delivered to the lungs.

Iodine solution of 0.5% or 1.25 % was found in studies to be inactivating coronavirus in 15 seconds but not by ethanol 70%, confirming what the povidone-iodine (povidone is the dispenser and iodine is the drug) manufacturers had asserted a couple of months ago. They had said a povidone-iodine (PVP-I) gargle or nasal spray could help in a big way. Iodine is a hundred-year old broad spectrum antiseptic which has come to have wider applications. Otherwise it is contraindicated in allergy and thyroid cases.

Iodine reduces the risk to the patient by decreasing the viral load going to the lungs by deactivating them at the primary bed of infection in the nasal mucosa region. A nasal decontamination by irrigation or spray under medical advice is also an adjunct to wearing of mask and reducing the intensity and volume of transmission to others via droplet and aerosol.

The confirmatory in vitro study was conducted at Connecticut School of Medicine, US.

SARS-CoV-2 initially infects ciliary cells of the nasal mucosa. (Cilia are tiny hair like structures on the surface of the ciliated cell facilitating sweeping or pushing things one way, for example, moving secretions down.) The virus then moves via the nasal-oropharyngeal axis to the lungs through microaspiration, leading to the damaging respiratory infections seen in COVID-19, according to JAMA.

It was studied that the variation in the features and severity of infection from patient to patient are caused by the differential propagation of the viral load from the nasal cavity to the lungs. The PVP-I helps here.

Since it rapidly inactivates SARS-CoV-2, it can also play an adjunctive role in mitigating viral transmission.

This study highlighted a virus transmission pathway that involves infection of ciliated cells of the upper airway within the nose as the dominant site of infection, followed by subsequent aspiration and seeding of the lungs. Mucociliary clearance was followed by aspiration of infected fluid into the lower airway.

The ciliated cells have the highest expression of angiotensin-converting enzyme 2 (ACE2), which is the main receptor for SARS-CoV-2. These ciliated cells with ACE2 expression were the cells most susceptible to infection. The infectivity of these cells was much higher than that of lower airway cells.
