Dental clinics, ideal for cluster transmission of corona, told to shut in containment zones

The Health Ministry has issued an advisory to dental clinics how to engage with patients as the clinics are ideal sources of cluster transmission of coronavirus epidemic. In red zones especially they must refrain from close contact with patients as the oral cavity and nostrils are the sources of infection of this asymptomatic pathogen. A teleconsultation must be done before calling patients to the clinics to minimise the interaction. Besides, the patients should fix appointments in order to reduce crowding.

NEW DELHI, May 21: The most vulnerable place for cluster transmission of coronavirus is the dental clinic. There is also no ideal occasion other than a dental procedure for the asymptomatic SARS- CoV-2 to spread out - a procedure that necessitates so much of proximity with one another and that in physical contact with other person's saliva, blood and respiratory track secretions for durations enough for sufficient number of corona-embedded particles to get attached on the other person for cross infection. An airconditioned clinic, especially, is also a vulnerable chamber for corona cluster transmission like what happened in a South Korean church or Indian BSF camps or some Mumbai police stations.

So here comes the guidelines from the Health Ministry that all the dental clinics in containment zones should shut, but can conduct teleconsultation. Those in red and orange zones must limit the interaction to short consultations after the patient has had teleconsultation on patient history and other details. Patients should take appointment online so that there is no crowding at the clinic, the notification said.

All emergency cases should essentially be referred to designated covid facilities. So also the dentists have been advised to defer routine and elective procedures. Oral cancer screening under the National Cancer Screening programme is deferred for now.

"Saliva is rich in COVID-19 viral load. Many patients who are asymptomatic may be carriers. For this reason, it is suggested that all patients visiting a dental office must be treated with due precautions," the notification said.

The clinics should see exhaust fan is in order, other fans are off and window ACs are serviced properly. Air cleaning systems of HEPA or higher standard or UV light filters may be used.

The death toll from coronavirus (covid) in India has gone far above the 3,000 mark, and the number of infections to over 1.12 lakh, and the count still alarmingly rising without a peak seen any time soon. Among the 180 countries infected by the ferocious coronavirus, India's ranking has gone up to 11, going by the number of confirmed cases, from the comfortable bottom level it occupied two months ago.
