What is sero test, is it really a useful tool to assess immunity against covid
Sero test may tell whether a person was earlier infected by covid virus by detecting antibodies. But since the antibodies can disappear after some time, sero test will be futile to rate the immunity in a community. An antigen test has a different objective. It is a diagnostic test done to find out if a person is currently infected. There are also nucleic acid- based tests such as PCR, which more accurately diagnoses the infection.

NEW DELHI: The commonly known corona test called sero test, or serological test, is an antibody test to find out whether a person had ever been infected by the covid virus sometime in the past, with or without getting the symptoms. That is, it is a testing to find out some signs left behind by an attack, or just signs of an entry of some virus dose into the body, possibly in the recent past. Normally, immunity (antibodies) is developed after 2-3 weeks of infection and that protects against future infections. However, the infection in the first time is tackled by the body's natural cells that fight the pathogen.

While sero test may tell whether a person was earlier infected by covid virus, an antigen (viral protein) test reveals if a person is currently infected with a pathogen such as the SARS-CoV-2 virus. Once the infection has gone, the antigen disappears. Then there are nucleic acid based tests such as PCR, which detect the presence of genetic material, and are foolproof in diagnosis.

Several States in India like Delhi, Maharashtra and Kerala are undertaking the sero survey in identified communities on a large scale to determine the level of immunity. It is done on the belief that it helps determine the extent of exposure to the pathogen.

When a pathogen enters our body, certain proteins are formed in the plasma cells in the blood which are like the white blood cells. These proteins are called antibodies. They become like the gatekeepers and fight any pathogen like virus or bacteria entering the human body.

The test to find out the presence of these antibodies' is the sero test. It is conducted through an enzyme linked assay (EIA).

An indigenous ELISA test kit was developed by the Indian Council of Medical Research (ICMR) and on May 30 it directed the States to undertake the tests in communities. Subsequently, in several districts the survey (test) is being done. These are only random sample surveys.

The ELISA and CLIA kits detect the quantity of antibodies present in the body while EIA says only detects the presence of antibodies. The ICMR also approved several other firms such as Zydus Cadila, J Mitra & Company, Meril Diagnostics, Voxtur Bio, Trivitron Healthcare, Karwah Enterprises, Avecon Healthcare,Roche Diagnostics, Abbott to produce their testing kits. Thyrocare, Metropolis, Suburban Diagnostics are some of the private chain labs who have started antibody testing.

Today several agencies are doing the test using various methods. At present, a large-scale survey is on in Delhi from August 1 to 5, despite criticism of its utility.

The WHO has said there is no guarantee about the accuracy of these sero tests. Some experts are of the opinion that antibodies are not always present, disappearing after about two months. Instead, a memory cell is formed that triggers production of antibodies when the virus attacks a second time. Lack of antibodies is not an indication of lack of immunity. Therefore the tests are no way to confirm if a person had been infected earlier, or has developed immunity.
