District Residency Programme for PG medicos to infuse life into
States' decaying medical services
ANALYSIS: The novel rotational District Residency Programme for PG
medicos will trigger a revolution in the medicare system in the
countryside. Because it will have far-reaching implications for the
healthcare delivery in India. Being rotational, it will be continuously
pumping fresh blood into the stagnant district medical services,
stinking since Independence. The medical services in the States will
be redefined. This will also be a welcome change for the medicos
unlike the earlier compulsory rural service scheme begotten by some
sick minds, which was a total failure ending in wastage of funds and
loss of precious services as nobody took it up.

NEW DELHI, Sept 27: The Centre has made it mandatory for all post-graduate
medical students pursuing MD or MS in the country to undergo a compulsory posting
for three months at the district hospitals in the States as part of their medical
course curriculum starting from the 2020-21 academic year.
The rotation, called District Residency Programme (DRP), will be in the 3rd
or 4th or 5th semester of the post-graduate programme. The DRP will be implemented
with the PG batch joining in 2020-2021. The student undergoing the training
will be called District Resident, the notification said.
The main objectives of the DRP are:-
To expose the post-graduate student to the district health system and involve
them in health care services being provided at the district hospital for learning
while serving;
To acquaint them with the planning, implementation, monitoring and assessment
of outcomes of the national health programmes at the district level;
To orient them to promotive, preventive, curative and rehabilitative services
being provided by various categories of healthcare professionals under the
umbrella of National Health Mission.
The district hospital should be at least 100-bedded with all facilities.
Satisfactory completion of the residency shall be an essential condition to
appear in the final examination of the post-graduate course.
The concerned State Government will provide appropriate amenities to the residents.
The residents will also get additional honorarium for their contribution to
the healthcare services of the State. They will continue to draw full stipend/salary
from their medical colleges.
The MCI's gazette notification has laid down the details of the programme.
The residents would be working under specialists at the district hospitals
and would be working in outpatient, inpatient, casualty and other areas, and
encompass night duties, PG medical of pre/para clinical disciplines (namely,
Anatomy; Biochemistry; Community Medicine; Forensic Medicine; Microbiology;
Pathology; Physiology; and Pharmacology) would be trained in and contribute
to the diagnostic/laboratory services, pharmacy services, forensic services,
general clinical duties, managerial roles and public health programs, etc.,
as applicable.
The district residents will serve in outpatient, inpatient, casualty and other
areas, and will do night duties. The district resident will work under the overall
directions and supervision of the District Residency Programme Coordinator (DRPC).
The programme will be coordinated jointly by the Department/Directorate of
Medical Education and the Department /Directorate of Health Services with the
MCI constituting a National Steering Committee to guide and oversee the programme.
The MCI has amended the Post-graduate Medical Education Regulations, 2000 for
the purpose.

Experts blame violation of the Visual Flying Rules (VFR) for most of the
chopper crashes in the mountains as the leading cause. VFR means fly only
visually - because of the lack of navigation aids in remote areas or unreliability
of GPS-like aids in hilly terrains or a remote manipulation of instruments
in cases of sabotage - as the topography plays havoc especially in the monsoon
(mid-June to September).
Even when VFR is followed, most crashes occur due to CFIT (Controlled Flight
into Terrain) - that is, everything else is OK but the aircraft collides with
hills or trees hidden by the clouds. This also occurs mostly in the monsoon.
On Tueday with Manang, the chopper was brand new, pilot was highly experienced.
But it hit an 'invisible' tree and then crashed against a hillside. This
CFIT occurred when the pilot tried to dodge patches of clouds even as he was
following the VFR.
Simply put, everyone knows flying in the hills during the monsoon is risky,
but the companies want to make more money in any which case. This type of
tragedy is very frequently invited in the Indian hill State of Uttarakhand
as well, where pilots are even made to fly without the mandatory rest.
